Before You Visit Our Office

 

On your first appointment with us, we will review the information provided by your referring dentist and your medical history and medications. We’ll also perform a clinical examination along with x-rays and periodontal charting. These steps help us create a personalized treatment plan for you.

To prepare for your first visit, please bring a complete list of your current medications and arrive 15 minutes early to allow time for us to collect and enter your information.

Financial and Insurance Policy

 

Payment of fees is due on the day of treatment. We accept cash, cheque, VISA, Mastercard, Amex and Debit payments.

Your insurance plan is an agreement between you and your insurer. You are responsible for the full cost of your treatment.

We will assist you with pre-determining which treatment fees your insurance policy will cover. If you have a pre-determination, you can assign your insurance benefits to your account with us and pay the balance owing on the day of treatment. If you don’t have a pre-determination, you will need to pay the full cost of your treatment and submit your insurance claim afterward for reimbursement. In some cases, we can provide pre-authorized and dated payments of your accounts, preferably by credit card.

 

Consultation Fees

 

A consultation fee applies to all new patients on their first appointment. This fee will also include any x-rays required during the examination. Additional fees will be applied for supplemental records for radiographs or models.
